Description

To ensure that patients newly diagnosed with HIV and/or as TB or STI symptomatic by Informal Settlement Program HAST counsellors present to the referral DOH facility timeously for further Management
Requirements

       Minimum Required Qualifications and Experience:

  • Matric (Senior Certificate)
  • 2 years’ experience in TB or HIV field
  • Computer Certificate

Desirable Qualifications and Experience: 

  • Previous qualification or experience in Adherence Counselling
  • Previous experience of working with Communities, Home Based Care, WBOT’s or Community Based Organisation

 

   Key Performance Areas

  • Ensuring that patients diagnosed within the community are linked to care and commenced on ART as applicable.
  • Improve Linkage to Care
  • Assist facility staff to implement services which will increase retention in care of clients initiated on ART
  • Networking
  • Compiling of statistics
  • Capturing of HIV patient data
  • General departmental support
